HELP!!! U.S. Certificate of Origin Needed

I have two turnings that are going to get shipped to Abu Dhabi, and I have been told I need to provide a Certificate of Origin.
The vendor sent a copy of one from China, as a sample, but it won't help me, and it LOOKS very official with seals and stuff.
I have no idea where to look for a certificate, anyone have any ideas?
ANY help would be appreciated.

George, I googled Certificate of Origin and about the 5th item down was one for exported goods. Sorry, but the chicken is burning!!!!!!!!!!!!!!!!!😀

George: One thing that you must keep in mind is that Abu may only accept certain certifying agencies to sign the form. Your buyer in Abu may be able to find that out for you. I've worked in several African countries and most of them insist that the certificate of origin be signed by a Chamber of Commerce in the exporting country. Canada, and I believe, the US have the same rules for exports from some countries. When we put NAFTA in, it was a big departure to let the exporter sign his own certificates of origin.

This is part of a contract I got through having a website. I have mentioned (talked about) it on another site, not so much to brag, as to encourage others to foster the same profile, you know, toot your own horn, don't hide your light under a bushel, etc.
In my "defense," I really did need the form, and I knew that someone on one of these forums would already have had the need for this information, and would be willing to share it.

But to be less stealth, it appears that you don't need the same paperwork to send things to Taipei - where the first three pieces went. 😀

Nah, George, go ahead and wave your flag, you deserve it.
I have pieces in England and Australia, and America, so flag waving is okay.
Besides, I am glad to have a copy of the form (thanks, Kurt), even though it was never asked for (yet).🙂
 
Shipping company as Broker

George,

I am assuming that to ship these pieces you are going to be using either DHL, FedEx or UPS. All three of these companies can act as your export broker and take care of all these papers for a few dollars. Just look up their contact number for international shipping on the web and contact them and they will know what you need for a country and can handle the entry forms needed.

George, Call the Embassy in DC. Ask for the Cultural Exchange office and lay the problem at his feet. These embassy folks should be "spring loaded" to help you as their country benefits in the end result. Phil
 
Nothing wrong with that, I have turnings in S.Africa and Germany.
 
Oh the days of ignorance is bliss 😱.... I have turned pieces in at least 8 countrys in N. & S. America, Europe and Asia (not a brag even a little bit, having seen so many wonderful photos of what you guys do, I feel like I'm sitting by a camp fire beating sticks on the ground and grunting) I have never had any type of request for doc of origin. Should I be worried or just wait until someone says something.

I would wait... If you need one, your local Chamber of Commerce seems to be the way to go to get it. I brought a document, they added two lines and a seal to it, haven't heard anything back from the retailer since, I assume everything is fine...
This was HER first encounter with the need for one. Apparently, most countries don't require them.
