Insert on Supernova 2 Chuck

Hello All:
Its been a while since I've been around - come to think of it its been a while since I've been a member or have been turning. Since COVID I've been home a lot and started back turning and even rejoined AAW. I forgot how much I enjoy it. I hope I can still make the time one we get back to normal and life goes haywire again.

Anyway since I've been back out in my wood shop and turning I've noticed a strange problem that is fairly annoying and probably dangerous. I have a Supernova 2 chuck on a JET 16-42 lathe. I have noticed lately that while using the chuck it periodically comes loose from the insert. At first I thought it was unscrewing itself off the lathe but as it happened a few times more its the insert between the lathe and the chuck to make it fir on my lathe that seems to be unscrewing itself.

I don't ever remember having this issue before and could use some advice on it as its kind of strange how this is happening because with the normal way the lathe spins, and me turning wood I would think the pressure of my tool on the wood would make it stay on tight.

I hope I can figure this out because I like the chuck, but don't want to have to keep stopping and tighten the insert back on the chuck every few minutes.

Thanks for everyone's thoughts on this.

There's a set screw that keeps the insert attached to the chuck. Perhaps yours is missing or loose. (There's also a teeny tiny little disc of gasket-like material that goes between the end of the set screw and the insert which disappears if you just look at it, but protects the insert from the set screw and maybe makes the screw stay in better)

Set screws on both the adapter and the chuck. I periodically check them while turning. Embarrassed to say but I didn't check the screw on the chuck. Fortunately I was out of the line of fire and had it on the lowest speed. I small dent in the bed rail serves as a reminder. Now I check everything that can be checked. Still, Joe, you are correct. When spinning in "forward" the chuck should tend to tighten on the insert and the insert should tend to tighten on the spindle.

Are you running in reverse at all?

Just sanding a little bit and maybe staining or putting on wax but not a whole lot. That's what kind of made me scratch my head.
